What is School Form 2 (SF2)?

The SF2 is a foundational document in the Department of Education (DepEd) reporting system. It tracks the daily attendance of learners and provides a monthly summary of:

• Average Daily Attendance
• Percentage of Enrolment for the Month
• Number of Learners Absent for 5 Consecutive Days (for dropout intervention)

Manually calculating these percentages for 40–50 students every month is prone to errors. Our automated version eliminates those risks.

How to Use the Automated SF2

Using the template is straightforward. Follow these steps to ensure your reports are error-free:

• Step 1: Download the file for the specific month you need.
• Step 2: Encode your learners' names (Last Name, First Name, Middle Name) in the designated column.
• Step 3: Enter the "School ID," "School Name," "Grade Level," and "Section" at the top.
• Step 4: For every school day, leave the cell blank for "Present" and encode "A" for Absent. The formulas will automatically calculate the totals.
